Manaus/AM – This Tuesday (17) is World Hypertension Day and the Department of Health of the State of Amazonas (SES-AM) warns of the risks and precautions for the prevention of the disease , which mainly affects noble organs such as the heart, kidneys and brain. Hypertension, known as high blood pressure, occurs when blood pressure measurement stays above 140 x 90 mmHg.
According to clinical cardiologist Abraão Nobre, who works at the Fundação do Coração Francisca Mendes Hospital, a unit of SES-AM, hypertension is one of the main causes of cardiovascular mortality and can be associated with other diseases, such as as diabetes, obesity, metabolic disorders. syndrome and harmful habits, such as a sedentary lifestyle, unhealthy diet and smoking.
“Hypertension, in general, in 90% of patients is called essential hypertension. Why? It means primary because the exact cause is not known”, specifies the specialist. “It is not necessarily a single gene that you have in your genome to cause hypertension, it is a family or multigenic disease. .”
Factors that provoke the development of hypertension can be divided into modifiable and non-modifiable. Among the modifiable factors are a sedentary lifestyle, caused by the lack of physical activity, and a poor diet with insufficient consumption of salt and sugars. The non-modifiable are sex, age and genetic heritage, which determine the predisposition to the development of the disease.
The cardiologist also mentioned other conditions that can lead to hypertension. “Regarding hypertension, 10% have a potential cause, for example, kidney disease, pheochromocytomas, which is a rare condition, being called secondary lateral hypertension. Foot swelling associated with high blood pressure during pregnancy could be Preeclampsia This, when associated with seizures, is eclampsia.
Diagnostic – The Ambulatory Blood Pressure Monitor (Map) is an automatic device, responsible for multiple blood pressure measurements throughout the day. During the examination, a map of the pressure is carried out in the patient, which is essential for the diagnosis of the disease.
“Another purpose of the card is for the patient, already on medication, who wants to optimize and increase or decrease (medication) intervals. For example, a patient who uses 100mg of certain medication in the morning and whose blood pressure will be high at night. We need to do a 24-hour mapping to better indicate the use of one or more drugs for the treatment of arterial hypertension”, reinforces the cardiologist.
Aid – The treatment of hypertension is carried out with the use of drugs prescribed for each patient, which reinforces the importance of consultation and follow-up with a specialized professional.
The cardiologist explains that the treatment takes place in three simultaneous stages: taking medication, changing lifestyle and physical exercise.
“Without one of them, the tripod doesn’t work. The patient must take his medication regularly and undergo periodic reassessment, which can be carried out by the general practitioner or by the cardiologist, depending on the severity of the case. The treatment is as if personalized. The patient may have the same age, weight and other comorbidities. Our body’s metabolism and responsiveness to drug treatment vary a lot, and that’s not “an automatic value” or a recipe for cake,” he stressed.
Service flow – The treatment must be initiated from the first visit to a Basic Health Unit (UBS). After this first medical evaluation, if a specialized follow-up is necessary, the patient will be referred to a unit in the secondary public network.
